At the time of registration you will need the following:
- Registration Forms
- State Birth Certificate (original needed, a copy will be made)
- Baptismal Certificate (if not baptized at St. David or Our Lady Help of Christians)
- Immunization Record
- Small picture of child
- Registration Fee of $100.00 per family
